Advanced Directives

Advanced Directives are commonly referred to as Living Wills or Dying Declarations. The purpose of an Advanced Directive is to give your health care providers, and those making your health care decisions, general direction on what type of end of your life care you desire if diagnosed with a terminable illness.

 

Do-Not-Resuscitate (DNR) – Our office does not prepare DNRs. DNRs must be signed in the presence of a care provider. These documents instruct your care providers to refuse resuscitation should you stop breathing.
